Requesting a copy of your Medical Records

If you require a copy of your medical records, please download and complete Kingston Hospital's "Subject Access Application". You can download a copy of the form here.  Each applicant will need to provide 2 forms of supporting identification (one from each category) and full details of this can be found in the application.  Unfortunately we cannot proceed with preparing the medical record until all the appropriate information has been received.   If you and your partner are both applying, please complete separate application forms and request the same information.   If the address you have provided on your application form has changed from the information in your medical record, please also provide previous address details, to avoid any delays in preparing your record.  Please provide a 'pen' signature on your application form.
